Fast Facts Sheet for CDA/CCP Advisors

This form was produced to help all of our approved Educators with the paperwork processes required for the services provided through the Oklahoma Training Approval System (OTAS) Educator Services. It is critical that these steps be followed to avoid any delays in services provided or in the invoicing process. Thank you, in advance, for your cooperation.

CDA Advisor/CCP Field Counselor Steps

STEP 1: Receive phone call from candidate. Take down all logistics and information as specified on the "Request for Service Approval Form" (pdf) Use one form per candidate per observation.

STEP 2: Refer to your individual "Scope of Work" included in your OTAS Educator Services packet. Be careful to note the pre-approval time of 15 days before the observation takes place.

STEP 3: Fax the completed "Request for Service Approval" form to (405) 799-7634, Attention: OTAS Educator Services

STEP 4: Receive in the mail the "CDA/CCP Assessment Observation Verification" form to be completed by you and the candidate and an invoice for the advisor to sign.

STEP 5: Once the service is completed, mail the completed "CDA/CCP Assessment Verification" form and the signed invoice to the OTAS Educator Services. No Faxes accepted.

STEP 6: Wait to receive payment within 6 to 8 weeks upon receipt, in our offices, of the invoice.
